Most of the time a clitoris that large is simply the result of an abnormal amount of testosterone being produced by the fetus.  It doesn't mean there's anything wrong with her genes.  She doesn't have an extra chromosome.  None of that.  There's even been some question as to if this has to do with maternal mitochondrial DNA (something that's just recently being considered due to abnormalities between "identical" twins), but I'm not going to quite count that as genetics yet.  However, it's very possible that she had a large clitoris in the first place, made larger by steroids.

I've done a lot of research into ambiguous genitalia (don't ask me why) and I've found that someone with XX chromosomes can even be born as a male due to the testosterone production in early pregnancy, with no chromosomal abnormalities.  Sometimes the testosterone production will even correct itself once it's regulated by the testes.  A man could have XX chromosomes his entire life and never know it.
